Does dry eye cause blurred vision?

Blurred vision from dry eye comes and goes: it improves when you blink and worsens when you focus. It is a sign that the surface of the eye needs attention.

Yes, dry eye is a very frequent cause of blurred vision, and it has a characteristic feature that helps identify it: the blurring is intermittent and tends to improve momentarily with blinking. Many patients describe it as vision that "clouds over and clears", that worsens at the end of the day or when focusing for a while, and that does not fully resolve no matter how much they rub their eyes or change their glasses prescription.

The explanation lies in the role tears play in vision. The tear film does not only lubricate the eye: it forms the first optical surface through which light enters, just in front of the cornea. For an image to focus clearly, that surface must be smooth and even. When there is dry eye, the tear film is insufficient or poor quality and distributes unevenly over the cornea, creating an irregular surface that distorts incoming light. The result is a blurred or fluctuating image.

Blinking is the key to understanding why this blurring comes and goes. Every time we blink, we spread a fresh layer of tears over the eye and the optical surface smooths out again for a few seconds, which is why vision clears immediately after blinking. As that tear film breaks up, evaporates or spreads unevenly, the image blurs again. This fluctuation is very characteristic of dry eye and helps distinguish it from other causes of blurred vision, such as a refractive error or a cataract, which produce a stable and constant blur.

Blurred vision from dry eye tends to be more noticeable in situations that reduce blinking or increase evaporation: when reading, working at a computer, looking at a phone, driving or spending time in air-conditioned or heated environments.

It is important to note that this blurring, although bothersome, does not represent a real or progressive loss of vision: it is a temporary effect of the ocular surface. Even so, it should not be ignored, as it is usually a sign that dry eye needs treatment before complications can arise. If you notice fluctuating blurred vision together with a sensation of dryness or eye discomfort, the recommended step is an ophthalmological assessment to confirm the cause and treat it accordingly.
